Regarding: Certified Community Behavioral Health Clinic PDI designation
Greeting All!
It is my esteem honor and privilege to announce that HRDI has been awarded a Program Development and Implementation (PDI) grant to become a Certified Community Behavioral Health Clinic (CCBHC) by the United States Substance Abuse and Mental Health Services Administration (SAMHSA). We were selected amongst nine peer organization across the state, of which three (including HRDI) are located within the City of Chicago.
What does CCBHC designation mean:
CCBHC distinction will permit us to expand the scope of our programs to a wider service area while impacting the lives of a larger population of clients, as we endeavor to ensure health equity and high quality care for the underserved and under sourced communities we serve.
CCBHC designation will also provide us an opportunity to further develop and enhance our workforce, facilities and equipment.
CCBHC scope of services fall under nine categories:
Screening, Assessment and Diagnosis
Patient Centered Treatment Planning
Outpatient Mental Health/Substance Use Disorder
Crisis Services
Peer Supports
Psychiatric Rehabilitation
Targeted Case Management
Primary Health Screening & Monitoring
Armed Forces and Veteran Services
CCBHC Timeline:
The award is for a period of four years, however continuation is based on accomplishing key program objectives. The first year of the award consist of creating a formal tactical approach to accomplishing milestones that are required as part of the Program Development and Implementation processes as a prelude to certification.
